该剧改编自休·豪伊所著同名畅销科幻系列小说，故事发生在一个反乌托邦未来，大约一万名人类在144层深的筒仓型地堡居住，他们被告知外面的世界因受到毒气感染而变得不适合人类居住，仅余的幸存者遂躲进地堡避难，并制订了规条来维持地堡中的新社会运作。

《小谢尔顿》衍生剧正在CBS运作中，将聚焦于蒙塔纳·乔丹和艾米丽·奥斯蒙特饰演的角色Georgie与Mandy。

蓝泽耕作（山下智久 饰）离开了翔阳大学附属北部医院的急救中心，成为了一名神经外科医生。白石惠（新垣结衣 饰）依然在人手不足的急救中心担当着领导者的角色。曾经也梦想成为飞行急救医生的绯山美帆子（户田惠梨香 饰）离开翔阳医院，成为了妇产科医生。冴岛遥（比嘉爱未 饰）依然从事着飞行急救护士的工作。藤川一男（浅利阳介 饰）与白石一起留在急救中心，成为了一名整形外科的急救医生。
